Minimum Requirements
- Associate, Bachelor's, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active Florida Physical Therapy license required prior to the start of the assignment.
- Home health care experience is required. Experience providing PT services in patients' homes, including documentation of services provided.
- Familiarity with home health travel routines and traveling between patient locations within a shift.
- BLS certification from AHA or ARC may be required.
- Preferred: HCHB (Homecare Homebase) EMR experience.
Assignment Details
- Facility type: Home Health.
- Shift: Days.
- Shift hours: 8:00 AM to 5:00 PM.
- Work days: Monday through Friday.
- Weekly guaranteed hours: 32 to 40.
- Productivity expectation: 30 points.
- Coverage area: Indian River County, St. Lucie County, and Martin County. Travel between patient homes is required. Mileage between visits will be reimbursed.
- On-call/weekend requirement: None.
- Required interview format: Teams.
- Dress code: Scrubs or slacks; any scrub color acceptable.
