Minimum Requirements
- Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
- Active Florida Physical Therapy License is required to start the assignment.
- BLS Certification from AHA or ARC may be required.
- Experience with outpatient settings preferred; candidate must be comfortable providing care across outpatient, assisted living facility (ALF), independent living facility (ILF), and skilled nursing facility (SNF) environments.
- Familiarity with Innova Health EMR preferred.
- Ability to manage productivity expectations, including SNF productivity targets (client expects approximately 75% productivity in SNF) and 45–60 minute outpatient treatment sessions.
- Willingness to work in a Continuing Care Retirement Community (CCRC) environment and to float between levels of care as needed.
- Open to new graduates. Plenty of support staff available.

- Facility Type: Outpatient with mixed CCRC settings (ALF, ILF, SNF).
- Shift Name: Days.
- Schedule: Monday through Friday, 9:00 AM to 5:00 PM.
- Patient Care Mix: Primarily outpatient with SNF responsibilities comprising a portion of caseload.
- EMR: Innova Health.
