Make a meaningful difference as a travel OT. We are seeking a dedicated Occupational Therapist to help patients in a skilled nursing facility regain or improve daily living skills while working in Ada, OK.
Minimum Requirements
- Bachelor's, Master's, or Doctoral degree in Occupational Therapy from an accredited educational program.
- Active Oklahoma OT license required to start the assignment. Compact license not accepted.
- Minimum 1+ years of occupational therapy experience is required.
- Experience in long term care (LTC) or skilled nursing facility rehabilitation is required.
- New graduates considered only if they have Level II fieldwork experience; Level I observation experience alone is not acceptable.
- Ability to perform standardized assessments and develop evidence-based plans of care for patients with cognitive and/or ADL-related physical impairments.
- Documentation competency in electronic medical records and accurate recording of skilled treatment minutes per regulatory requirements.
- Ability to supervise and support OT assistants, rehabilitation aides, and students as needed.
- BLS certification from AHA or ARC may be required prior to start. (Preferred: include if available.)
- Required submission materials: resume, skills checklist within 1 year, and 0–1 reference at time of submission.
Assignment Details
- Facility type: Skilled Nursing Facility.
- Shift: Days.
- Schedule: 8 hour days, 35 GWW (full-time weekly hours).
- Duration: 13 week assignment.
- Weekend requirement: None.
- Candidates must have an active OT license at time of submission.
Key Responsibilities
- Screen, evaluate, and treat patients with cognitive and/or ADL-related physical deficits, following regulatory and clinical practice standards.
- Administer interventions such as ADL retraining, therapeutic activities, therapeutic exercises, modalities, and use of assistive devices as indicated.
- Utilize standardized assessments and evidence-based practice to guide treatment planning and to measure outcomes.
- Document assessments, treatment plans, progress notes, and discharge summaries accurately in the electronic documentation system.
- Adjust treatments based on patient progress and refer to other providers as appropriate.
- Communicate effectively with supervisors, interdisciplinary team members, patients, families, and caregivers.
- Participate in required meetings including patient care conferences, utilization review, and rehabilitation meetings.
- Obtain necessary DME and supplies as clinically indicated.
