Minimum Requirements
- Current State License as a Physical Therapist in Arizona is required to start the assignment.
- Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program is required. Doctoral or Master’s degree acceptable if from an APTA-approved program.
- Proof of current CPR certification is required.
- Current driver’s license, reliable transportation, and insurance are required due to home health travel requirements.
- 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting is required.
- Previous home care experience is preferred.
- Ability to perform clinical assessments, develop and revise Plans of Care, and document progress notes per agency standards.
- Ability to lift, bend, push, pull and perform prolonged standing and walking as job duties require.
- Ability to work a flexible schedule and travel locally as needed.
- Must read, write, and speak fluent English.
Assignment Details
- Facility Type: Home Health.
- Assignment Type: Travel contract.
- Start Date: May 2026 (assignment length through August 2026).
- Primary Role: Provide physical therapy assessments and treatment for neuromuscular, neurological, and orthopedic conditions in patients of all ages in home settings.
- Clinical responsibilities include preparing and revising Plans of Care, documenting progress notes and phone orders, educating patients and families, and coordinating with interdisciplinary teams.
- Must be able to order and adapt durable medical equipment and instruct patients in safe use of assistive devices.
- May require local travel between patient homes. Reliable transportation is required.

Additional Job Information
