Minimum Requirements
- Degree in Physical Therapy (Bachelor’s, Master’s, or Doctoral) from an accredited program approved by APTA.
- Active Michigan Physical Therapy license required to start the assignment.
- BLS certification from AHA or ARC may be required.
- Must have at least 12 months of Home Health experience within the past 3 years (client requirement).
- Experience with HCHB (Homecare Homebase) EMR is required (client requirement).
- Ability to manage a travel territory including multiple ZIP codes: 49707, 49777, 49766, 49747, 49744, 49753, 49766, 48742, 48721, 48740.
- Ability to pass any required background and credentialing checks.
- Preferred: prior experience in rural home health settings and familiarity with Medicare home health documentation requirements.

- Shift Details: Days, 8-hour shifts, Monday through Friday.
- Schedule: Monday - Friday, 40 hours per week.
- Length of Contract: 13 weeks.
