Facilitates care of patients and families by providing them with a comprehensive continuous plan of care.
Responsibilities:
- Patient and Family Services: Identification of patient and families requiring coordination of community support services by screening medical records, attending team meetings and referrals from other clinical staff. Develops treatment plans and facilitates services to enhance adjustment to illness and maintenance of wellness.
- Medical Case Management Services: Identification, planning and case management, that alleviates environmental stresses and assures optimal success of medical. Identifies at risk population and utilizes case management areas to enhance continuity of care and wellness. Manages referrals to Department of Children and Families, Hospice and other institutions as necessary.
- Crisis Management: Responds expeditiously to emergency needs of patients and/or families by providing appropriate interventions necessary to stabilize, improve or resolve crisis situations during regular working hours demonstrates the ability to manage crisis situations effectively and under pressure. Maintains positive interactions with referring facilities, agencies, clinic staff and works to alleviate stressful situations for patient and families.
- Documentation: Substantiates services rendered by documenting in the medical record and maintains accurate current Clinic Information System Records and submitting other documentation as needed.
- Professionalism: Enhances the quality of patient care by complying with assurance standards, policies and procedures of the hospital and he department adhering to NASW code of Ethics.
- Administrative Duties: Attends required meetings, participates in committees and focus groups; completes required reports in a timely manner.
- Age-Specific Competency: Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served. The individual must demonstrate knowledge of the principles of growth and development over the life span and must possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his or her age-specific needs, and to provide the care needed as described in the department's policies and procedures.
- Provides psychotherapy services to clinic patients on a referral basis. Provides assessment, diagnosis and treatment of mental disorders, behavioral problems and stressful life events that affect patients. Develops evidenced based treatment plans to help patients overcome acute or chronic distress.
- Contributes to the efficient operation of the clinic by performing other related duties as may be required
Qualifications:
Minimum Education and Experience Requirements:
- Master's Degree in Social Work from an institution accredited by the Council on Social Work Education.
- Current Florida professional licensure as a social worker.
- Three to five years' recent clinical experience (applicable to designated population preferred).
- LCSW Required.
Motor Vehicle Operator Designation: Employees in this position: Will operate vehicles for an assigned business purpose
Licensure/Certification/Registration: LCSW required.
