Minimum Qualifications
- Valid and unrestricted Physical Therapist license in the state required to start the assignment.
- Bachelor’s, Master’s, or Doctorate in Physical Therapy from an accredited program.
- BLS certification from AHA or ARC may be required.
- Experience in skilled nursing or long-term care preferred.
- New graduates welcome.
- Strong interpersonal and communication skills required for collaboration with interdisciplinary teams and patient/family education.
- Ability to perform thorough evaluations, develop individualized treatment plans, document care, and adjust plans based on patient progress.
Preferred Qualifications
- Experience working with geriatric populations in a skilled nursing or LTC setting.
- Familiarity with common therapeutic modalities used in post-acute and long-term care.

Impacting Patient Care
Conduct thorough evaluations of patients’ physical abilities and limitations. Develop and implement personalized treatment plans and utilize therapeutic techniques and modalities to facilitate recovery. Collaborate with interdisciplinary teams including physicians, nurses, and occupational therapists to optimize care. Educate patients and families on rehabilitation goals and home exercise programs. Maintain accurate and timely documentation and remain current with industry best practices and regulatory requirements.
