Physical Therapist — Home Health — Pomona, CA
Looking for a change of scenery and a chance to help others. Provide skilled physical therapy services in a home health setting to patients recovering from injury or managing chronic conditions. This opportunity is located in Pomona, CA and supports clinicians seeking a community-based practice environment.
Minimum Qualifications
- Doctoral, Master’s, or Bachelor’s degree in Physical Therapy from an accredited program approved by the APTA.
- Active California physical therapy license required prior to start of assignment.
- At least one year of home health physical therapy experience required.
- BLS certification from American Heart Association or American Red Cross may be required.
- Valid driver’s license and reliable transportation for patient visits in the community.
- Experience with HomeCare HomeBase (HCHB) is preferred but not required.
Preferred Qualifications
- Prior travel or per diem home health experience preferred.
- Experience working with post-acute care or complex medical populations preferred.
Assignment Details
- Shift hours: 8:00 AM to 5:00 PM.
- Schedule: Monday through Friday, 8-hour shifts.
- No on-call or weekend requirements.
- Caseload includes patients in Pomona and surrounding communities such as Yorba Linda, Orange, Anaheim Hills, and Santa Ana.
