Looking for a change of scenery and a chance to help others. Provide skilled physical therapy services to patients in their residences across Newton and nearby communities. Manage patients with physical impairments, functional limitations, or disabilities resulting from injury, disease, or other causes. Collaborate with patients, families, physicians, and allied health professionals to identify needs and coordinate services. Maintain thorough and timely electronic documentation and interdisciplinary communication in accordance with agency policies and standards of practice.
Minimum Requirements
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active Massachusetts Physical Therapist professional license required prior to start.
- Minimum 1 year certified home health experience required. Experience must be within a certified home health agency; hospice, private care, or standalone case manager roles do not satisfy this requirement.
- Minimum 1 year case management experience.
- BLS certification may be required from AHA or ARC.
- Ability to travel to patient cases as assigned.
Preferred Qualifications
- Experience working with a broad patient age range, from pediatrics to geriatric populations.
- Familiarity with electronic clinical documentation systems used in home health settings.
Assignment Details
- Shift Hours: 0800-1630.
- Weekend rotations occur approximately once every six weeks.
- Ability to travel to cases as assigned within the service area, including Lynn, Nahant, and Saugus.
