This position supports skilled home health patients with a primarily geriatric caseload and uses HCHB for charting. Travel clinicians must have reliable transportation due to occasional long-radius visits.
Minimum Qualifications
- Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active California PT license required prior to the start of the assignment.
- At least 1 year of recent skilled home health physical therapy experience. Experience must include at least one year within the last three years.
- Experience using HCHB electronic medical record required.
- OASIS experience required for clinicians performing start-of-care visits.
- Current BCLS (Basic Cardiac Life Support) certification required.
- Traveler must have own transportation.
Preferred Qualifications
- Home health experience with geriatric caseloads preferred.
- Experience managing up to 6 patient visits per day and meeting productivity standards (30 productivity points standard) preferred.
Assignment Details
- Shift hours: Monday through Friday, 8:00 AM to 5:00 PM.
- Caseload: Geriatric patients. May visit up to 6 patients per day.
- Productivity standard: 30 productivity points standard.
- Electronic charting: HCHB required.
- Occasional long drive radius possible; routes may extend to 50+ miles.
