Minimum Requirements
- Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program, or equivalent degree required.
- Current State License as a Physical Therapist in PA or ability to obtain before start of assignment required.
- Proof of current CPR (BLS) certification required.
- Current driver’s license, reliable transportation, and active vehicle insurance required.
- 1-2 years of experience as a Physical Therapist in an acute care or rehabilitation setting required.
- Previous home health experience preferred.
- Ability to perform prolonged standing, walking, heavy lifting, bending, pulling, and pushing as part of patient care required.
- Ability to travel locally and work a flexible schedule required.
- Must read, write, and speak fluent English.

- Job Duties: Provide evaluations, develop and revise Plans of Care, document progress notes and phone orders, instruct patients and families on transfers and equipment, order rehabilitative equipment, and collaborate with interdisciplinary teams.
- Skills Required: Knowledge of durable medical equipment; manual dexterity and hand-eye coordination; ability to supervise other care team members when applicable.
