Provide physical therapy in patients' residences to address physical impairments, functional limitations, or disabilities resulting from injury, disease, or other causes. Collaborate with patients, families, physicians, and allied health professionals to identify needs and coordinate services. Utilize timely electronic documentation and interdisciplinary communication in accordance with agency policies and standards of practice.
Minimum Qualifications
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active Massachusetts Physical Therapist license required prior to start of assignment.
- Minimum 1 year of home health experience within a certified home health agency required.
- BLS certification from AHA or ARC may be required.
- Ability to travel to patient cases as assigned within the service area.
- Weekend rotations required approximately once every six weeks.
Preferred Qualifications
- Experience with electronic clinical documentation systems used in home health settings.
- Experience providing care to a broad age range, from pediatric to geriatric patients.
