Minimum Requirements
- Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active Massachusetts Physical Therapy license required to start the assignment.
- BLS certification from AHA or ARC may be required.
- Minimum 1 year of certified home health experience in a certified home health agency.
- Minimum 1 year of case management experience.
- Ability to travel to assigned patient cases within the service area.
Preferred Qualifications
- Experience working with electronic medical records and timely electronic documentation.
- Experience collaborating within interdisciplinary home health teams.
Assignment Details
- Shift Hours: 08:00–16:30.
- Weekend rotations occur approximately once every six weeks.
- Ability to travel to cases as assigned across regional service areas.
