Provide skilled physical therapy to patients in their homes to promote recovery from injury, improve mobility, and manage chronic conditions. Travel to assigned cases across the region and collaborate with interdisciplinary teams while maintaining thorough electronic documentation and case management responsibilities.
Minimum Requirements
- Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active Massachusetts Physical Therapy license required to start the assignment.
- BLS certification from AHA or ARC may be required.
- Minimum 1 year of certified home health experience in a certified home health agency.
- Minimum 1 year of case management experience.
- Ability to travel to assigned patient cases within the service area.
Preferred Qualifications
- Experience working with electronic medical records and timely electronic documentation.
- Experience collaborating within interdisciplinary home health teams.
Assignment Details
- Shift Hours: 08:00–16:30.
- Weekend rotations occur approximately once every six weeks.
- Ability to travel to cases as assigned across regional service areas.
