Minimum Requirements
- Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
- Active Texas Physical Therapy license required to start the assignment.
- BLS Certification from AHA or ARC may be required.
- Home health experience required. Must have experience with home health evaluations, start-of-care visits, 30-day reassessments, treatment visits, and discipline discharges.
- Familiarity with HCHB electronic medical record preferred. Candidates with HCHB and home health experience will be prioritized.
- Ability to work as an independent contractor and document per agency standards.
- Minimum education requirement: Bachelor’s Degree.

- Shift schedule: Days 08:00–16:00 (client also lists 08:00–16:30 consistently as expected daily schedule).
