Overview:
The position is more accurately described as an Evaluation Coordinator that serves as the primary clinical coordinator for patients undergoing evaluation for advanced heart failure therapies, including heart transplantation and LVAD implantation. Working collaboratively with physicians and the multidisciplinary care team, this role guides patients through the evaluation process from initial referral through Selection Committee review. Responsibilities include patient and caregiver education, obtaining informed consent for evaluation, coordinating required testing and consultations, preparing comprehensive clinical summaries for committee presentation, and collaborating with financial coordinators to secure necessary authorizations for listing or device implantation. The Evaluation Coordinator is also responsible for managing transplant waitlist activities, maintaining accurate documentation within transplant regulatory databases, and ensuring compliance with applicable regulatory and program requirements throughout the patients journey to transplantation or implant.
Responsibilities: Participates in the development, implementation, and maintenance of the left ventricular assist device (LVAD) program and the heart transplant program; to coordinate and manage the patient care of this high-risk patient population throughout the continuum. - Graduate from a nursing program Required
Work Experience- 2 years of professional clinical experience Required
- Cardiovascular focus experience Preferred
- 1 year experience caring for LVAD patients and heart transplant patients Preferred
Licenses and Certifications- RN - Registered Nurse - Georgia State Licensure and/or NLC/eNCL Multistate Licensure Required
- ACLS Required
- BCLS - Basic Life Support Required
