Minimum Requirements
- Associate, Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active Kentucky PT license or ability to obtain state licensure prior to start.
- Current CPR/BLS certification.
- Experience with Home Health setting, including start-of-care evaluations, recertifications, and discharges.
- Familiarity with Home Health documentation systems such as HCHB or PointClickCare/Pointcare.
- Ability to travel within coverage areas: Boyle, Garrard, and Madison counties.
- Willingness to work assigned shift schedule (see Assignment Details).
- Minimum education requirement: Associate degree.
- Preferred: Prior home health travel experience and familiarity with mileage reimbursement processes between visits.

- Days: 5x8 schedule.
- Coverage Areas: Boyle, Garrard, and Madison counties.
- Productivity Expectations: 30 points (client-specific).

- On-call/Weekend Requirement: No call requirement.
- Mileage reimbursed between visits from first patient to last patient.
