Minimum Requirements
- Associate's, Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
- Active Kentucky Physical Therapist license required to start the assignment.
- BLS certification required.
- Experience with OASIS documentation.
- Experience with HCHB or PointClickCare (PointCare) electronic documentation.
- Home health experience preferred.
- Minimum education requirement: Associate degree (client-specific).
Assignment Details
- Primary specialty: Physical Therapist.
- Facility type: Home Health.
- Shift: Days.
- Shift hours: Monday–Friday 0800–1600 (client notes indicate Monday–Friday 0800–1630 EST schedule and Days 0800–1600).
- Weekend/on-call requirements: One weekend per month and one weekday on-call per week.
- Coverage areas: Madison, Montgomery, Clark, Powell counties.
- Typical responsibilities: SOC, evaluations, recertifications, ROC, agency discharges within scope of practice.
- Mileage reimbursed between visits from first patient to last patient.
- Floating to other facilities is not indicated. Follow facility assignment as directed.
