Minimum Requirements
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active Massachusetts Physical Therapist license required to start the assignment.
- BLS certification from AHA or ARC may be required.
- Minimum 1 year of certified home health experience. Roles in hospice, private care, or non-certified case management do not meet this requirement.
- Minimum 1 year of case management experience within a certified home health agency.
- Ability to travel to patient cases as assigned within the service region.
- Weekend rotations approximately once every six weeks.
- Preferred: experience with electronic clinical documentation and interdisciplinary care coordination.
