Minimum Requirements:
- Active pharmacist license in the state of Iowa. Must be registered with the Iowa Board of Pharmacy.
- RPh or PharmD degree required.
- BLS (Basic Life Support) certification required.
- Minimum 2 years hospital pharmacy experience preferred. Client preference for candidates with hospital experience; CAH experience not required.
- Experience with EPIC electronic health record highly preferred.
- Knowledge of federal and state pharmacy laws, medication therapy monitoring, sterile compounding, and medication dispensing procedures.
- Ability to compound sterile IV products, including hazardous products, and oversee Omnicell use and security.
- Ability to participate in rapid response, code blue, and trauma calls during business hours and provide scheduled after-hours on-call coverage.

Standard Job Description:
Provide clinical pharmacy services that support safe medication use for hospitalized patients. Serve as a clinical resource for providers, nurses, and other patient care personnel. Create vancomycin dosing regimens and monitor lab levels as needed. Compound or oversee compounding of sterile IV products including hazardous products. Dispense and maintain prescription records in each patient record and provide age-appropriate pharmaceutical services. Offer verbal counseling to outpatient clients or proxies at the time the initial prescription leaves the pharmacy. Document serious or uncommon adverse drug reactions and summarize interventions in the electronic medical record. Manage initial contacts from pharmaceutical representatives. Participate in antibiotic stewardship and pharmacy & therapeutics committees. Oversee Omnicell operations including staff access, stock levels, and dispensing. Participate in rapid response, code blue, and trauma calls during business hours. Provide scheduled after-hours on-call pharmaceutical services.
