Minimum Requirements
- Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active West Virginia Physical Therapist license required to start the assignment. Compact PT license accepted.
- BLS certification required from the American Heart Association or American Red Cross.
- Home health experience required, including knowledge of OASIS assessments and homecare documentation practices.
- Experience developing exercise programs and providing rehabilitation interventions in home health settings.
- Preferred: Experience with home health charting systems, ideally HomeCare HomeBase (HCHB).

Impacting Patient Care
Perform physical therapy evaluations and treatment based on physician orders. Complete comprehensive evaluations using appropriate tools and establish measurable goals and treatment plans to promote patient independence and function. Reassess progress regularly and modify plans as needed. Communicate patient needs and progress to medical providers, families, and the care team. Recommend and assist in securing adaptive equipment. Maintain documentation for evaluations, treatment, and billing according to company and regulatory requirements. Supervise PT assistants and aides within scope of practice. Participate in staff development and maintain required continuing education.
