Minimum Requirements
- Associate, Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active Kentucky PT license or eligible to obtain prior to start of assignment.
- Current CPR/BLS certification from AHA or ARC.
- Experience with home health documentation systems; HCHB or PointClickCare (PointCare) experience required.
- Prior home health physical therapy experience preferred. Minimum experience in start of care, evaluations, recertifications, and discharges preferred.
- Ability to provide care across assigned coverage counties (Boyle, Garrard, Madison) and travel between patient homes. Mileage reimbursed between visits from first patient to last patient.
- Ability to comply with client dress code: gray, black, blue scrubs or business casual.
- Associate degree minimum education requirement accepted per client guidance; higher degrees accepted.
