Minimum Requirements
- Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
- Active Pennsylvania Physical Therapy license required to start the assignment.
- BLS certification from AHA or ARC may be required.
- Experience in home health setting preferred. This includes familiarity with home visit workflows and patient safety in home environments.
- Experience with electronic documentation systems preferred, specifically HCHB and OASIS familiarity is preferred.
- Valid driver’s license and ability to travel within assigned counties is required.
- Minimum education requirement: Bachelor’s Degree.
Assignment Details
- Facility type: Home Health.
- Shift: Days. Schedule: Monday through Friday day shifts.
- Weekly hour guarantee: 32 hours per week.
- Coverage areas: Mifflin County and Juniata County. Candidate must travel between patient homes within coverage area.
- Mileage reimbursement between visits from first patient to last patient is provided.
- On call/weekend requirement: No.
- Required certifications: Active PT license.
- Dress code: blue scrubs.
