Minimum Requirements
- Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program, or equivalent accredited degree (Master’s or Doctoral acceptable).
- Current State License as a Physical Therapist in good standing and active prior to the start of the assignment.
- Proof of current CPR certification required.
- Current driver’s license, reliable transportation, and vehicle insurance required.
- 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting required.
- Ability to perform clinical assessment, develop and revise Plans of Care (POC), and document progress notes and orders per agency policy.
- Experience providing treatment for neuromuscular, neurological, and orthopedic disorders required.
- Ability to instruct patients and caregivers in transfer techniques and safe use of durable medical equipment; knowledge of durable medical equipment required.
- Ability to lift, bend, push, pull, and perform prolonged standing and walking consistent with home health duties.
- Ability to work a flexible schedule and travel locally as needed.
- Must read, write, and speak fluent English.
- Previous home care experience preferred.

- Scope: Provide physical therapy services to patients of all ages in home settings, including assessment, POC development, treatment, education, and documentation.
- Team: Collaborate with interdisciplinary team and consult with physicians as required.
- Mobility: Must have reliable transportation for home visits and local travel between patient homes.
