Minimum Requirements
- Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program.
- Active Oklahoma state license required prior to the start of assignment. Compact license is not acceptable.
- At least 1 year of Physical Therapist clinical experience is required. New graduates are acceptable only if they have completed a Level II clinical experience rather than solely Level I observation.
- Experience in long term care (LTC) or skilled nursing/rehab settings is required.
- BLS certification may be required from AHA or ARC.
- Ability to perform standardized assessments, wound care, therapeutic exercise, manual techniques, ambulation training, use of assistive devices, and document treatment minutes per regulatory requirements.
- Ability to supervise PT assistants, rehabilitation aides, or students as needed.
- Adherence to APTA and state practice acts, code of conduct, and professional ethics.
- Preferred: prior experience with electronic clinical documentation systems.

Position Overview
Provide comprehensive physical therapy services for residents in a skilled nursing environment. Responsibilities include screening, evaluation, developing and implementing plans of care, delivering interventions such as manual techniques, therapeutic exercise, modalities, ambulation training, and wound care, and documenting outcomes in the clinical record. Communicate effectively with interdisciplinary team members, participate in care planning and facility meetings, and provide discharge summaries per regulatory requirements.
