Minimum Requirements
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active physical therapy license required or willingness/ability to apply for Iowa license prior to starting. Candidates without an Iowa license will be considered if they are willing and able to obtain Iowa licensure.
- Minimum 1.5 years of licensed Physical Therapist experience required.
- Home health experience highly preferred.
- Experience treating all age groups required.
- Epic electronic medical record experience required.
- BLS (Basic Life Support) certification required (AHA or ARC acceptable).
- Driver's license required.
- Work history/resume, skills checklist completed within one year, license verification, copy of required certifications, and one supervisory reference required at time of submission.
- Candidates must not have been an employee of the facility within the last 12 months.
- Must have a taxable address greater than 50 miles from the facility unless otherwise noted by the client. (Local candidate allowances and exceptions are client-determined.)
- Preferred: prior home health field equipment experience and prior travel therapy experience.
Assignment Details
- Facility Type: Home Health.
- Shift: Days.
- Schedule: 5x8 hour shifts, 40 hours per week.
- Shift Hours: Beginning at 8:00 AM.
- No call.
- No weekends required.
- EMR: EPIC.
- Orientation: Weekly, every Monday.
- Float Requirement: Candidate may be asked to float within the same discipline across local campuses.
