Minimum Requirements

Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
Active New York State Physical Therapy license required to start the assignment.
BLS certification from AHA or ARC may be required.
Experience providing care in a skilled nursing facility or long-term care setting is preferred.
Prior experience with restorative programs, mobility training, transfers, and fall prevention is preferred.
Ability to document using electronic medical records. Experience with SNF workflow and therapy documentation systems is preferred.
