Minimum Requirements
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active Ohio Physical Therapy license required to start the assignment.
- BLS certification from AHA or ARC may be required.
- Home Health experience is required.
- Experience with HCHB (Homecare HomeBase) electronic documentation is required.
- Ability to manage a caseload consistent with weekly productivity expectations (points-based productivity; client indicates 32 points weekly).
- Valid driver’s license and reliable transportation for travel within assigned territory (Guernsey County and eastern Muskingum County).
- Preferred: prior territory-based home health experience and comfort with community-based care coordination.
