Minimum Requirements
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active South Carolina Physical Therapist license required to start the assignment.
- BLS certification from the American Heart Association or American Red Cross may be required.
- Home health experience required. Experience with start-of-care visits, reassessments, OASIS documentation, and homebound patient populations preferred.
- Demonstrated ability to complete in-home patient care; expectation to provide at least 20 units of in-home patient care each week during the assignment.
- Ability to meet physical demands of home health practice including frequent driving, walking, standing, bending, lifting up to 50 pounds, and safe operation of clinical equipment.
- Reliable transportation and willingness to travel within assigned county service areas, including Clarendon, Sumter, Lee, Darlington, and Florence counties.
- Clinical documentation accuracy and compliance with federal and state home health regulations required. Knowledge of Medicare Conditions of Participation preferred.
- Must be able to pass required background checks and meet agency onboarding requirements.
Assignment Details
- Facility Type: Home Health.
- Shift Name: Days.
- Shift Hours: 08:00–17:00.
- Assignment Length: 13 week assignment.
- Service Area: Coverage may include Clarendon County, Sumter County, Lee County, Darlington County, and Florence County; travel between patient homes is required.
- Productivity Expectation: Minimum expectation of at least 20 units of in-home patient care per week.
- Orientation: Up to 2 weeks of orientation paid at an equivalent of 30 units.
- Documentation and visit types performed: Routine PT visits, PT start of care, PT evaluations, PT reassessments, PT supervisory visits, OASIS recertification/resumption, transfers, and discharge visits.
