Behavioral Health Care Manager (LMSW/LCSW) – SNF | CoCM Program
Overview:
We are an established SNF physician group seeking a Behavioral Health Care Manager (BHCM) to support our Collaborative Care Model (CoCM) program across Skilled Nursing Facilities (SNFs) in Georgia. This role focuses on managing residents with depression, anxiety, and other behavioral health conditions through a structured, team-based model in partnership with primary care providers and a consulting psychiatrist.
This is a high-impact, population health role centered on resident engagement, care coordination, and outcomes tracking.
Responsibilities:
Conduct resident outreach and follow-up (primarily telephonic/virtual; coordinate with on-site SNF staff)
Administer and track PHQ-9, GAD-7, and other screening tools
Maintain a behavioral health registry and monitor clinical outcomes
Provide brief, evidence-based interventions (behavioral activation, supportive counseling, motivational interviewing)
Collaborate closely with SNF nursing staff, PCPs, and care teams
Present cases during weekly psychiatric case reviews and implement recommendations
Track and document time and services in compliance with CoCM billing requirements
Ensure accurate, timely, and compliant EMR documentation
Qualifications:
Active Georgia license as:
Experience in behavioral health, SNF/long-term care, or care management preferred
Strong communication and interdisciplinary collaboration skills
Ability to manage a high-volume caseload
Comfortable working across multiple facilities and care teams
Proficiency with EMRs and documentation workflows
