Minimum Requirements
- Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program, or equivalent accredited degree.
- Current state license as a Physical Therapist in Oregon required to start the assignment.
- Proof of current CPR certification required.
- Current driver’s license, reliable transportation, and vehicle insurance required. Local travel between patient homes is expected.
- 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting required.
- Previous home health or home care experience preferred.
- Ability to perform prolonged standing, walking, heavy lifting, bending, pulling, and pushing as part of patient care responsibilities.
- Ability to read, write, and speak fluent English.
- Ability to work a flexible schedule and travel locally.

Job Summary and Responsibilities
- Provide skilled physical therapy services to patients of all ages in the home setting, including assessment, plan of care development, implementation, and ongoing evaluation.
- Document and maintain clinical records, including Plan of Care, progress notes, and phone orders in compliance with regulatory and agency standards.
- Plan and deliver treatment for neuromuscular, neurological, and orthopedic conditions based on medical referral and clinical assessment.
- Train patients and caregivers in safe transfer techniques and use of assistive devices such as walkers, wheelchairs, canes, braces, and prosthetic appliances.
- Order appropriate durable medical equipment and adapt equipment to home environment limitations when indicated.
- Provide patient and family education tailored to age, culture, and readiness to learn.
- Follow infection control practices and universal precautions during patient care.
- Supervise and coordinate other team members when applicable and collaborate with the interdisciplinary team and physicians.
