Minimum Requirements
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active Michigan Physical Therapist license required to start the assignment.
- Current CPR certification required.
- Valid driver's license, vehicle insurance, and access to a dependable vehicle.
- Home health experience required. Clinical experience in home health settings preferred.
- Experience using Homecare Homebase (HCHB) electronic medical record required.
- Ability to evaluate functional status, develop plans of care, and document per CMS and agency guidelines.
- Schedule flexibility required, including participation in weekend rotation and potential availability on observed holidays.
- Ability to travel within the agency service area and visit patients across Oakland, Wayne, Macomb, Livingston, Genesee, and Washtenaw counties as needed.
