Minimum Requirements
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active Ohio Physical Therapy license required prior to start of assignment.
- Current BLS/CPR certification from the American Heart Association or American Red Cross.
- Valid driver’s license, vehicle insurance, and reliable transportation for home health visits.
- Experience working in home health or other outpatient/ community-based settings preferred.
- Demonstrated ability to perform evaluations, develop treatment plans, and document patient progress per plan of care.
