What You'll Do:

We are seeking a dedicated Discharge Planner– LPN or Social Workto join our long-term care team and play a vital role in ensuring safe, smooth, and person-centered transitions from admission through discharge.

Key Responsibilities of a Social Worker:

Manage an assigned caseload from admission through discharge

Develops discharge plans and documents interventions and pertinent information in medicalrecordsona timelybasis

Conduct psychosocial assessments and develop individualized careand discharge plans

Provide counseling, emotional support, and strong advocacy for residents and families

Collaborate with the interdisciplinary team,participatein care conferences, andfacilitatefamily meetings

Connect residents with community resources and arrangeappropriate post-discharge services

Provide crisis intervention and address behavioral or emotional changes

Serve as a resourceforstaff and families andparticipatein ongoing professional development

Rate of Pay: USD $30.00 - USD $30.00 /Hr. -:

What You'll Bring:

Qualificationsof a Discharge Planner:

Previous experience in long-term care, geriatrics, or a related healthcare setting
Knowledge of discharge planning processes and community resources

Strong assessment, documentation, counseling, and crisis intervention skills

Excellent communication and interpersonal abilities

Commitment to person-centered care and resident advocacy
Must have a LSW, LSWA or LPN license
