What You'll Do:
We are seeking a dedicated Discharge Planner– LPN or Social Workto join our long-term care team and play a vital role in ensuring safe, smooth, and person-centered transitions from admission through discharge.
Key Responsibilities of a Social Worker:
- Manage an assigned caseload from admission through discharge
- Develops discharge plans and documents interventions and pertinent information in medicalrecordsona timelybasis
- Conduct psychosocial assessments and develop individualized careand discharge plans
- Provide counseling, emotional support, and strong advocacy for residents and families
- Collaborate with the interdisciplinary team,participatein care conferences, andfacilitatefamily meetings
- Connect residents with community resources and arrangeappropriate post-discharge services
- Provide crisis intervention and address behavioral or emotional changes
- Serve as a resourceforstaff and families andparticipatein ongoing professional development

What You'll Bring:
Qualificationsof a Discharge Planner:
- Previous experience in long-term care, geriatrics, or a related healthcare setting
- Knowledge of discharge planning processes and community resources
- Strong assessment, documentation, counseling, and crisis intervention skills
- Excellent communication and interpersonal abilities
- Commitment to person-centered care and resident advocacy
- Must have a LSW, LSWA or LPN license
