Ovation Hospice continues to grow, and we are seeking a compassionate and experienced Nurse Practitioner to join our Denver hospice team.
As a Nurse Practitioner with Ovation Hospice, you will play a vital role in supporting patients and families through one of life’s most meaningful journeys—providing comfort, dignity, and expert clinical care at the end of life.

Responsibilities
- Provide comprehensive hospice services in compliance with state and federal hospice regulations and the state Nurse Practice Act
- Perform face-to-face visits to evaluate hospice eligibility and ensure continued appropriateness for hospice care
- Collaborate with the Interdisciplinary Group (IDG) to develop and update individualized plans of care
- Serve as a medical resource to clinical and administrative staff, providing expertise in pain and symptom management
- Ensure appropriate medical measures are provided to control symptoms and promote patient comfort
- Conduct thorough assessments, review clinical information, and oversee patient care in coordination with attending physicians and the IDG
- Participate in team conferences, care planning, and performance improvement initiatives
- Maintain accurate and timely clinical documentation in accordance with regulatory requirements
- Demonstrate exceptional communication and counseling skills with patients, families, and colleagues
- Stay current on best practices in hospice, palliative care, and symptom management
Requirements
- Current and unrestricted Nurse Practitioner license in the state of Colorado
- Completion of an accredited Nurse Practitioner program
- Minimum of 2 years of hospice, palliative care, or geriatric experience preferred
- Controlled substances registration with state and federal authorities (if applicable)
- Strong ability to communicate effectively and compassionately with patients and families
- Valid driver’s license and ability to travel throughout the Denver area to patient homes and care settings
- Must not be excluded from participation in the Medicare program
