JOB SUMMARY
Ensures that all services are delivered in accordance with the Mission Statement and S.P.I.R.I.T. Value of Carroll Hospice and Carroll Hospital Center. Provides professional, comprehensive, family-oriented nursing care to individuals in the Inpatient setting. Assesses patients' and families' needs and formulates, implements and evaluates plan of care. Performs primary care case management function of assigned patients.
REQUIREMENTS
- Required Associate's Degree ]
- Preferred Bachelor's Degree
- 2-4 years Requires minimum of two years of recent medical/surgical nursing experience. One year home health or hospice experience preferred. Must have expertise in performing health assessments. Required
- CHPN - Certified Hospice Palliative Nurse
- CPR - AHA Healthcare Provider
SPECIFIC REQUIREMENTS
Interpersonal communication skills (written and oral) required in order to educate and comfort patients and their families; main effective contacts with a variety of hospital personnel; maintain appropriate documentation. CHPN certification should be within 4 years of hire or after meeting CHPN exam requirements of 500 hours of hospice and palliative nursing practice in the most recent 12 months or 1000 hours of hospice and palliative nursing practice in the most recent 24 months (required to apply for CHPN exam).
