Minimum Requirements
- Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
- Active State License is Required to Start the Assignment
- BLS Certification May Be Required from AHA or ARC
- Physical Therapist license in Indiana required to start.
- HEP B required.
- TB required.
- Flu required.
- Tdap required.
- Criminal Background required to start.
- OIG Search required to start.
- Statement of Good Health required to start.
- OSHA Packet required to start.
- Verification of Professional Education required to start.
- I-9 Form required to start.
- GSA Search required to start.
- UDS - 9 Panel (Marijuana Not Included) required to start.
- Contract Therapist Compliance Attestation required to start.
Assignment Details
- Monday and Wednesday, 8:00 AM–5:00 PM, with the last patient at 4:00 PM.
- Tuesday and Thursday, 10:00 AM–7:00 PM, with the last patient at 6:00 PM.
- Friday, 7:00 AM–4:00 PM, with the last patient at 3:00 PM.
- One-hour lunches are included.
- Documentation system: NextGen.
- Floating to another clinic may be considered if staffing is secured.
- Patient population includes outpatient orthopedic patients.
