Minimum Requirements
- Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
- Active State License is Required to Start the Assignment
- BLS Certification May Be Required from AHA or ARC
- Current unrestricted license
- Doctoral education required
- OASIS visits experience required
- Add-on evals experience required

- Schedule: Monday-Friday, 8:00 AM-4:30 PM
- Weekend requirement: None
- Coverage area: Kanawha County
