The Medication Access Coordinator within the Health System plays a vital role in facilitating patients' access to various financial resources for medications.
- Evaluating the financial circumstances of uninsured or under-insured patients, ensuring adherence to regulatory compliance, and upholding patient confidentiality and dignity.
- Determine eligibility for medication assistance or any available financial aid, collaborating closely with the Clinical Team to optimize the organization's efforts in mitigating unnecessary financial losses linked to high-cost medications.
- Acts as a patient advocate in resolving issues related to medication payment and coverage. To assess potential qualification for Medication Assistance Programs and Foundations, the coordinator conducts thorough patient interviews, reviewing health and prescription insurance, financial status, and required documentation.
- Actively seeking clinical information from patient charts is a key aspect of the coordinator's responsibilities.
- Effective communication with representatives of assistance programs, as well as with patients, physicians, pharmacists, and other relevant stakeholders, is crucial for both the maintenance of patients in existing medication assistance programs and the initiation of new programs.
