What You’ll Do
As a Home Health RN, you’ll provide skilled, compassionate care by:
Completing comprehensive patient assessments, admissions, and ongoing care planning.
Coordinating care with physicians and the interdisciplinary healthcare team.
Delivering skilled nursing services, including therapeutic, preventive, and rehabilitative care.
Monitoring patient progress and updating individualized plans of care.
Educating and supporting patients and their families throughout their healthcare journey.
Maintaining accurate, timely clinical documentation while ensuring compliance with regulatory standards.
What You’ll Need
Current Registered Nurse (RN) license.
Graduate of an accredited nursing program.
Minimum of one (1) year of clinical nursing experience (ICU, Med-Surg, or Rehabilitation preferred).
Home Health experience preferred, but not required.
Current CPR certification.
Valid driver’s license, reliable transportation, and current automobile insurance.
