Minimum Requirements:
- Must have State License to Apply
- EPIC experience
- Experience with medication reconciliation for discharge medication list
- Communication skills for gathering prior to admission home medication list from patient, family, or pharmacy
- Communication skills for discharge counseling with patients/families
- Clinical knowledge of antimicrobial stewardship
- Interpretation of culture results for recommendations
- Knowledge of UTI, pneumonia, SSTI, and related treatment recommendations
- Experience with pharmacokinetic dosing management and monitoring
- Experience with anticoagulation dosing management and monitoring
