What You'll Do
As an LPN, you will play a vital role in ensuring the health, safety, and well-being of our participants by:
- Providing direct nursing care and health monitoring
- Conducting health assessments and contributing to individualized care plans
- Administering and documenting medications accurately
- Coordinating medical, dental, and specialty appointments
- Serving as a liaison between participants, families, pharmacies, and healthcare providers
- Monitoring medication administration practices and conducting quality assurance reviews
- Responding to participant health concerns and emergency situations
- Maintaining accurate medical records and regulatory compliance
- Training and supporting staff on medication administration, infection control, and safety procedures
- Educating participants on wellness, treatment plans, and self-care practices
- Collaborating with community partners and healthcare professionals to ensure continuity of care
- Supporting participants with daily activities as needed
What We're Looking For
- Current Licensed Practical Nurse (LPN) license in Massachusetts
- At least one year of nursing experience preferred
- Experience working with adults, seniors, or individuals with disabilities is a plus
- Current CPR and First Aid certification
- Strong communication, organizational, and clinical skills
- Bilingual candidates, especially those fluent in Cape Verdean Creole, are strongly encouraged to apply
