You will help patients recover from injuries and manage chronic conditions while delivering care in patients' homes. Minimum Qualifications
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active CT Physical Therapist license required prior to the start of the assignment.
- Minimum 1 year of home care experience required.
- BLS certification from AHA or ARC may be required.
- Experience conducting approximately 6 patient visits per day in a home health setting.
- Familiarity with home health documentation and workflows.
Preferred Qualifications
- Working knowledge of HCHB EMR software preferred.
Assignment Details
- Shift hours: 0700 - 1530.
- Weekend rotation required.
- Typical daily caseload: approximately 6 patient visits per day.
