Looking for a change of scenery and a chance to help others. Provide physical therapy services to patients in their place of residence who have physical impairments, functional limitations, or disabilities resulting from injury, disease, or other causes. Collaborate with patients, families, physicians, and other allied health professionals to identify needs and coordinate services. Utilize timely electronic documentation and interdisciplinary communication in accordance with agency policies and standards of practice for home health care. Travel to patient homes as assigned within the service area.
Minimum Requirements
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active Massachusetts Physical Therapist professional license required to start the assignment.
- At least 1 year of certified home health care experience. Experience in hospice, private care, or standalone case management does not qualify for this requirement. Only roles within a certified home health agency are acceptable.
- At least 1 year of case management experience.
- BLS certification from AHA or ARC may be required.
- Ability to travel to patient homes and cases as assigned within the service area.
Preferred Qualifications
- Previous experience with electronic clinical documentation systems in home health settings.
- Familiarity with interdisciplinary coordination in home-based care.
Assignment Details
- Shift hours: 0800-1630.
- Weekend rotations occur approximately once every 6 weeks.
- Ability to travel to cases as assigned within the region.
- Primary service locations include Lynn, Nahant, and Saugus, MA.
