Physical Therapist - Home Health (Pomona, CA)
Seeking a Physical Therapist to provide home health services in Pomona, CA. The clinician will evaluate, plan, and deliver therapeutic interventions to patients in their homes to improve mobility, reduce pain, and promote independence. Candidates must be skilled in documentation, care coordination, and patient education within a home health setting. Prompt responses are appreciated.
Minimum Qualifications
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active California Physical Therapy license required to start the assignment.
- At least one year of home health (HH) experience required.
- BLS certification from AHA or ARC may be required.
- Ability to travel to patient homes throughout assigned service area including Corona, Riverside, Moreno Valley, Norco, Eastvale, Jurupa Valley, Perris, and Redlands.
- Reliable transportation and valid driver’s license required for home visits.
Preferred Qualifications
- Experience with HomeCare HomeBase (HCHB) electronic documentation preferred, not required.
Assignment Details
- Shift hours: 8:00 AM to 5:00 PM.
- Work days: Monday through Friday.
- No on-call or weekend requirements.
- Caseload will include visits across multiple cities within the regional service area; candidate must be willing to travel within that area.
