Minimum Qualifications
- Degree in Physical Therapy (Bachelor’s, Master’s, or Doctoral) from an accredited program approved by the APTA.
- Active Massachusetts Physical Therapy license required to start the assignment.
- BLS certification from AHA or ARC may be required.
- Minimum 1 year of certified home health experience in a licensed certified home health agency.
- Minimum 1 year of case management experience.
- Ability to travel to patient cases as assigned.
Preferred Qualifications
- Experience with electronic health record documentation in home health settings.
- Experience performing admissions and interdisciplinary care coordination in home health.
Assignment Details
- Shift Hours: 0800-1630.
- Weekend rotation: once every 6 weeks.
- Ability to travel to assigned cases within the service area (includes Chelsea, East Boston, Revere, Winthrop).
