Looking for a change of scenery and a chance to help others. Provide skilled physical therapy services in home health settings for patients across the lifespan. Plan, implement, and evaluate individualized plans of care based on medical referral and clinical assessment. Collaborate with the interdisciplinary team to optimize patient function, provide family education, recommend and adapt durable medical equipment, and document care per regulatory and agency requirements.
Minimum Requirements
- Current state Physical Therapist license valid for practice in Massachusetts. Required.
- Bachelor of Science, Master of Science, or Doctoral degree in Physical Therapy from an APTA‑approved program. Required.
- Current CPR certification. Required.
- Current driver’s license, reliable transportation, and personal auto insurance. Required.
- 1–2 years of experience as a Physical Therapist in an acute care or rehabilitation setting. Required.
- Home health experience. Preferred.
- Ability to perform patient handling tasks including lifting, bending, transferring, and prolonged standing and walking. Required.
- Ability to travel locally and float between patient locations as needed. Required.

- Previous home care experience. Preferred.
