You will evaluate and treat patients in their homes based on physician orders. Responsibilities include completing comprehensive evaluations, developing measurable goals and individualized treatment plans, reassessing progress and modifying treatment as needed, documenting evaluation and treatment in required formats and timelines, coordinating with physicians and interdisciplinary team members, recommending and assisting with adaptive equipment, and supervising PT assistants and aides as allowed by the state practice act. You will follow applicable professional practice standards, company policies, HIPAA, and state and federal regulations.
Minimum Qualifications
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active Kentucky Physical Therapy license required. Compact license accepted.
- Experience in home health or home-based care preferred.
- Medicare OASIS experience preferred.
- BLS certification from AHA or ARC may be required.
- Experience supervising Physical Therapist Assistants and aides preferred.
Preferred Qualifications
- Previous travel therapy experience preferred.
- Familiarity with adaptive equipment assessment and procurement preferred.
Assignment Details
- Shift Hours: 08:00 AM to 05:00 PM.
- Work Days: Monday through Friday.
- County coverage includes Daviess, Ohio, McLean, Henderson, and Webster.
