Travel Physical Therapist — Home Health, Huntington, WV
Seeking a traveling Physical Therapist to provide home health services in Huntington, WV and surrounding counties. The clinician will evaluate, plan, and deliver therapeutic interventions to patients in the home setting. This role requires collaboration with interdisciplinary teams, documentation of patient progress, and adherence to agency policies and state regulations. Candidates should be prepared to work independently in community settings and manage a caseload across rural and suburban areas.
Minimum Qualifications
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active WV state Physical Therapy license or eligible to obtain licensure prior to start.
- Home health experience required.
- Experience with HCHB (Homecare Homebase) required.
- BLS certification from AHA or ARC may be required.
- Minimum education requirement: Bachelor’s Degree.
- Ability to travel within assigned coverage areas (Putnam, Cabell, Wayne counties).
Preferred Qualifications
- Prior travel physical therapy experience preferred.
- Familiarity with home health documentation workflows and interdisciplinary care coordination preferred.
- Comfortable using electronic documentation systems in field settings.
Assignment Details
- Shift hours: 0800–1600 (Monday–Friday schedule noted as Mon–Fri 8:00 AM–4:30 PM in client notes).
- Work schedule: 8 hours per day, 5 days per week.
- On-call/weekend requirement: No.
- Coverage areas include Putnam, Cabell, and Wayne counties.
- Dress code scrub color: navy blue.
