Minimum Requirements
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active Massachusetts Physical Therapist license required to start the assignment.
- BLS certification from AHA or ARC may be required.
- Minimum 1 year of certified home health experience within a certified home health agency.
- Minimum 1 year of case management experience.
- Ability to travel to cases as assigned.
Preferred Qualifications
- Experience with electronic documentation systems used in home health settings.
- Prior experience working with interdisciplinary home care teams.
Assignment Details
- Shift Hours: 0800-1630.
- Weekend rotations occur once every 6 weeks.
- Ability to travel to patient homes across assigned service areas.
